The Fund Request form is used to request program-related funds PRIOR to purchase through the 20 Pearls Foundation, in alignment with the collaborative fundraising structure with the Theta Theta Omega Chapter of Alpha Kappa Alpha Sorority, Inc.
Please complete this form before any funds are spent to ensure proper review, approval, and compliance with Foundation financial processes.
The Program Impact form ensures we capture impact, outcomes, and lessons learned to strengthen future programming and maintain compliance with donor, community, and Foundation requirements.
Please submit this form within 30 days after the event/program.
